New Patient Registration

registrationThe doctors welcome new patients  who live within our practice area. 

Patients register with the practice and are able to see the doctor of their choice. In the interest of continuity, we assign each new patient to a “usual” doctor and would suggest that you consult with this doctor for any ongoing problems.

When you register with us you will be asked to complete our registration forms and a supplementary information form. You will need one piece of photograghic ID and one proof of address to complete your registration. It is important to complete all forms in full so that your medical record can be brought up to date immediately. (It can take several weeks for your record to arrive from your previous doctor).
